Online Membership Application Form

To subscribe to the Kingston MultiSport Membership using EventsOnline.ca please follow these steps:

  1. Type your name and other information into the form below and click on the submit button.
  2. Use your Visa or Mastercard credit card to pay for your entry. The transaction will be processed securely in Canadian dollars.
  3. Once you have successfully paid for your entry your information will automatically be added to the event database and the the Eventsonline.ca confirmation page for the event will be updated. Depending on your Internet connection, this process may take about 16 seconds. During this time do not click your Browser's back button or stop button.

  • Active Member:
    A club member intending to regularly come to training sessions requiring the concurrent purchase of insurance provided by OAT(Ontario Association of Triathletes) and attend social and club functions. Cost: $50.00/yr. Please provide 2009- OAT # below
  • YOU MUST JOIN OAT TO BECOME A KMS ACTIVE MEMBER!!!
  • Apply/Renew for OAT Membership online at http://www.triathlonontario.com

  • KMS Supporter:
    A club member wishing to support the many services and programs provided by the club and may attend social and club functions, but will not participate in club workouts. KMS Supporters may attend up to two club trial training sessions annually, but must sign an OAT insurance waiver each time. Not eligible to participate in the Annual Club Tri due to lack of OAT insurance. Cost: $45.00/yr.

WAIVER/RENONCIATION:

IN CONSIDERATION of the acceptance of my application for registration as a member of the Kingston MultiSport Inc. (KMS) I, for myself, my heirs, executors, administrators, successors and assigns HEREBY RELEASE, WAIVE AND FOREVER DISCHARGE KMS and its respective agents, officials, employees, contractors, representatives, successors and assigns OF AND FROM ALL claims, demands, damages, costs, expenses, actions, and causes of actions, whether in law or equity, in respect of death, injury, loss or damage to my person or property HOWSOEVER CAUSED, arising or to arise by reason of my registration as a member of KMS or my participation in any KMS sponsored and/or sanctioned event whether prior to, during or subsequent to any such event AND NOTWITHSTANDING that some may have been contributed to or occasioned by the NEGLIGENCE of any of the aforesaid. I also hereby agree and consent to the use of my name or image by KMS. I agree to obey by the Highway Traffic Act during all KMS events and understand that I must wear a helmet that is CSA or ANSI approved during all KMS cycling or multisport events.
