In consideration of allowing me/ my minor child to participate in the program, related events and activities of Mackatak Coaching and Kevin Mckinnon, personal trainer. I Warrant That: I am familiar with the physical risk of serious injury and death, I believe I am physically, emotionally and mentally able to participate in the program, I understand the rules of participation and will remove myself if I feel unable to continue participation. I Understand and Agree on behalf of myself, my minor child, my heirs and next of kin that my participation in this program and execution of this document constitutes: An unqualified Assumption of All Risks associated with participation, a Full and Final Release and Waiver of Liability of Mackatak and Kevin Mckinnon, personal trainer and any and all persons or organizations associated with it or the program, an Understanding Not to Sue the releasees for any loss, injury costs or damages, and an Agreement to Indemnify the releasees from any and all litigation expenses, legal fees, liability damage, award or cost.
