DECHARGE
Je comprends que le sport du ski de fond renferme des risques inhérents qui
pourraient me causer des dommages corporels, voire la mort.
Considérant que Ski de fond Canada (désigné ci-après par SFC), Ski de fond
Québec (désigné ci-après par Division d’accueil) et le Sport Organisation
Laurentides (désigné ci-après par le club d’accueil) acceptent mon
inscription aux activités nommées ci-dessous, je conviens, au nom de
moi-même et au nom de mes héritiers, de mes liquidateurs de succession, de
mes administrateurs et mes ayants droit, de renoncer de façon irrévocable à
tout réclamation, réparation civile, coût (incluant les frais d'avocat et de
client à plein tarif), action en justice, action pour cause ou procédure ou
de quelque autre nature qui pourrait être intenté contre SFC, son conseil
d'administration, ses agents, ses commanditaires, ses partisans, ses
employés, ses bénévoles et le Parc national d’Oka, à la suite de tout perte,
accident ou blessure, y compris la mort ou des dommages, subi à cause de
négligence de la part de SFC, ses divisions et/ou ses Clubs affiliés et/ou
tout terrain de ski, ses agents, ses commanditaires, ses partisans, ses
employés et ses bénévoles.
Sans limiter le caractère général du susdit, je renonce à toute action ou
réclamation présente ou future qui pourrait être intentée suite à une
décision prise par SFC, par la Division d’accueil ou par le Club d’accueil.
En plus du susdit, je renonce à toute réclamation qui pourrait être intentée
suite à l’annulation ou la reprogrammation de l’événement pour n’importe
quelle raison.
Je déclare que mon état physique, pour autant que je sache, est adéquat pour
me permettre de participer sans incident au sport du ski de fond, et
qu'aucun médecin ou autre individu accrédité me m'a déconseillé de
participer au sport. En outre, je reconnais et j'accepte que c'est à moi
seul de décider si je me soumets à un examen clinique avant de participer au
sport, et par la présente je reconnais et j'accepte tous les risques
associés au choix de ne pas me soumettre à un tel examen ou, si je me
soumets à un tel examen et qu'on me conseille de ne pas participer au sport,
j'accepte tous les risques découlant de ma participation à ce sport.
Par la présente, je consens à ce que SFC publie tout matériel dans lequel
figure mon nom et/ou mon image, et je renonce à toute réclamation que je
puisse avoir contre SFC ou contre tout individu ou organisme sous son égide,
suite à la publication originale ou des publications subséquentes dudit
matériel.
Par le fait de compléter et signer cette Décharge et abandon de
responsabilité, j’accepte d’obéir aux règlements établis par SFC, par la
Division d’accueil et par le Club d’accueil, et d’obéir aux instructions
données par les officiels au cours de cet/ces événements.
DÉCLARATION
Par le fait de compléter et de signer cette Décharge et abandon de
responsabilité, je déclare mon acceptation des règlements tels qu'établis
par SFC et FIS, qui en font une exigence obligatoire pour tout détenteur de
licence; j'accepte de participer et de me conformer au Programme canadien
d'antidopage (PCA) ainsi qu'à la Politique SFC sur le dopage (document
1.10). Je déclare connaître le contenu et les dispositions du PCA relatifs
aux violations des règlements concernant l'antidopage. Je déclare connaître
les pénalités prévues dans l'éventualité d'une violation des règlements
d'antidopage et je comprends que le refus de fournir un échantillon d'urine
et/ou de sang, selon les modalités prescrites par le PCA, est considéré
comme une violation des règlements d'antidopage. Pour de plus amples

WAIVER
I recognize that the Sport of Cross Country Skiing has some inherent dangers
that could cause me bodily injury including death.
In consideration of Cross Country Canada (hereinafter called CCC), Ski de
fond Québec (hereinafter called the Host Division) and SClub de ski de fond
parc du Mont-Orford (hereinafter called the Host Club) accepting my entry
into the above named activity, I hereby for myself, my heirs, executors,
administrators and assigns, forever release, and forever discharge, CCC, the
Host Division and the Host Club, their executive directors, servants,
agents, sponsors, supporters, employees, volunteers or “ le Parc national
d’Oka from any and all claims, demands, damages, costs (including solicitor
and client costs on a full indemnity basis), actions or causes of actions,
proceedings arising out of or in consequence of any loss, injury or damage
which may arise by reason of negligence of CCC, the Host Division and the
Host Club, their servants, agents, sponsors, supporters, employees or
volunteers.
Without limiting the generality of the foregoing, I further release any and
all recourses which I may now or hereinafter have resulting from any
decisions of CCC, the Host Division and the Host Club. In addition to the
foregoing, I further waive any claims I might have in connection with any
cancellation or rescheduling of the event for whatever reason.
I declare that my physical condition, to the best of my knowledge, is
adequate to participate safely in the sport of cross country skiing, and
that no physician or other qualified individual has advised me against
participating in the sport. I further acknowledge and agree that it is my
choice as whether I will obtain a physical examination prior to
participating in the event, and I hereby acknowledge and agree to assume all
risks associated with not obtaining such examination, or if I do obtain an
examination and is instructed not to participate in the event, I hereby
assume all risks associated with my participation.
I authorize and consent to the publication by CCC, the Host Division and the
Host Club of any materials containing my name or picture, and I release to
CCC, the Host Division and the Host Club and all persons acting under
authority from them, any claims I might have due to the initial or
subsequent publication of such material.
By completing this Waiver of Liability with my signature below, I hereby
agree to abide by the Rules and Regulations as set forth by CCC, the Host
Division and the Host Club, and to follow the instructions of the officials
during the event.
DECLARATION
By completing this Release and Waiver of Liability with my signature below,
I hereby agree to abide by the Rules and Regulations as set forth by the CCC
and FIS, and as a requirement for license-holders, the undersigned hereby
agrees to participate in and be subject to the Canadian Anti-Doping Program
(CADP) and document 1.10 CCC Anti-Doping Policy. The undersigned is fully
aware of the doping violations and consequences rules set out in the CADP.
The undersigned is also fully aware of the penalties that will be imposed in
the event of an ant-doping rule violation and understands that refusal to
submit to sample collection as required under the CADP is considered an
